Important INFORMATION FOR PARENTS
Medical Information Forms We encourage parents to securely store important health and contact information online in your recreation account for each child registered. Doing this once, saves time and effort for parents, particularly if registered in multiple camps. Log in using
Pick-Up Times Please ensure that you are aware of pick-up times. In case of late pick-up, please inform your town centre. Indicate on the Medical Disclosure and Consent form if your child (6 years and older) has permission to leave camp unaccompanied. Persons listed on your authorized pick-up list may be asked to provide identification at pick-up.
Tips for What to Bring to Camp
Healthy “nut-free” lunch and snacks Hat Sunscreen Appropriate footwear (no flip-flops) Change of clothes Swimsuit and towel (if applicable) Water bottle
your MySurrey account email and password. Please notify your camp supervisor if your child: has medical or physical conditions has extra support in school
uses prescribed and/or non-prescribed medication including an inhaler or Epi-Pen (must include a note from doctor for non-prescribed medication).
Financial Support & Information Financial Assistance is available through Child Care Subsidies for licensed programs: www.gov.bc.ca/childcare. Our extended and licensed preschool programs may be eligible for additional subsidy. Contact your local recreation centre for more details.
Adapted Support One-to-one or shared support is available in camp for participants with disabilities. Request for support here by following steps below. Support requests are subject to space availability and support resources and will be reviewed upon registration. REQUEST FOR SUPPORT PROCESS 1. Choose the two weeks you are requesting support. 2. Complete the registration questionnaire and select the appropriate level of support your child requires, One to One OR Shared . 3. Adapted Camps Youth on the Move Adapted Camp An engaging and inclusive program that utilizes the City of Surrey's accessible bus to participant in community outings and local attractions.

Participants will receive support at either a 1:2 or 1:1 staff-to-participant ratio, depending on individual needs and spot availability. HOW TO REGISTER Visit surrey.ca/register and select Youth on the Move. Complete the registration questionnaire. Please select support ratio option here. Staff will contact you via email if a spot is available. The program will open with 3 available spots. Please waitlist as additional spots will be opened after registration, dependent on participant needs and staff ratios. Email inclusion@surrey.ca for more information.
