HEALTH AND SAFETY SERVICES

Assertive Community Treatment Assertive Community Treatment (“ACT”) teams provide person-centered, recovery oriented, flexible, community-based outreach mental health services for adults with serious and persistent mental illness that makes it difficult to manage their daily living. These individuals often have difficulty connecting with, or responding well to, traditional outpatient mental health and rehabilitation services that are generally offered in an office setting. Surrey has two ACT teams, providing specific care for Surrey and Surrey/North Delta. Services are available for adults who are 19 years or older, who have serious and persistent mental health and complex needs, are at risk of further deterioration and/or risk of homelessness and who may have had multiple visits to hospital and/or frequent police/ambulance services. Referrals can be made via mental health teams, community service providers, and hospitals.

Community Bed-Based Services

These programs are best suited for people with moderate to severe substance use issues and would include: • Having increasing severity of substance use. • Trying non-bed based substance use services without success. • Having low levels of social supports, including unemployment or under employment. • Having significant family/relationship challenges. • Experiencing multiple relapsing events following substance use treatment. This regional program has services located directly in Surrey. Referrals can be completed by Fraser Health or Fraser Health funded services, via clinicians and counsellors (including acute social workers).

Bed-Based Mental Health Services The mental health supported living program assists in providing recovery oriented services in a stable and supportive living environment for individuals with mental health challenges. These settings enable individuals to develop and/or maintain their social and life skills to work towards the highest level of independence possible. These services may provided by contracted service agencies. This regional program has services located directly in Surrey. Access is for individuals connected to the Fraser Health mental health centre. Bed-Based Substance Use Services Bed-Based Treatment services address the physical, emotional, social, intellectual, and other needs of each person who enters substance use treatment in a short or long term live-in setting.
